Disabilities Care Management

Care Management at Jewish Family Services assures that individuals with developmental disabilities and severe and persistent mental illness are given the opportunity to achieve their fullest potential. We provide a continuum of services for adults, where Care Managers can assist with every part of life, including:

  • Create an individualized care plan
  • Help with government benefits and entitlements such as SSI, SSDI, Food Share and T19
  • Assist with affordable housing
  • Coordinate services including home care
  • Provide Representative Payee and money management services
  • Schedule and arrange medical appointments
  • Socialization opportunities
  • In house links to counseling
  • Information, referrals and advocacy

These services offer adults who have developmental disabilities or severe and persistent mental illness a higher level of care and support. Services are provided in the home and community. Please call 414-390-5800 for more information.
