Care Management for Families

JFS Exceptional Needs Care Management provides a collaborative process of assessment, planning, facilitation and advocacy. Care management serves as a means for achieving client wellness and autonomy through communication, education, identification of service resources and service facilitation.

The underlying premise of our care management services is that when an individual reaches their optimum level of wellness and functional capability, everyone benefits: the individuals being serviced, their support systems, the health care delivery systems and our local community.

How JFS Can Help

  • Create an individualized care plan
  • Help with government benefits and entitlements such as SSI, SSDI, Food Share and T19
  • Assist with affordable housing
  • Coordinate services including home care
  • Provide Representative Payee and money management services
  • Schedule and arrange medical appointments
  • Socialization opportunities
  • In house links to counseling
  • Information and referral for issues including power of attorney for healthcare

* It is important to note that this program does not interfere with entitlement to governmental program/subsidies.

Getting Started

A trained Jewish Family Services staff member will complete a formal assessment with your family. This confidential assessment includes an extensive personal history, discussion of developmental, physical and emotional disabilities, family traditions, and discussion regarding expected future outcomes.

What Happens Next?

A customized care plan is created which includes arrangements for Jewish Family Services to provide, coordinate, and monitor the agreed upon services. The plan can be initiated in-whole or in-part at any time, or be scheduled to begin after a parent or caregiver's death. The assessed fees are commensurate with the services necessary to maintain the healthiest and safest lifestyle for your loved one.
