LinkAges

LinkAges is a comprehensive care management program designed to meet the needs of older adults and their families. These needs may include personal independence, health, social connectedness, nutrition and psychological well-being.

To help identify the right level of assistance one needs, a JFS care manager will conduct an assessment and then discuss the options, resources and alternatives available.

Available Services

  • Needs evaluation
  • Preparation of individualized care plans to optimize health and well being
  • Ongoing monitoring and evaluation for continuity of care
  • 24 hour coverage, 7 days a week
  • Liaison for families at a distance
  • Medical management
  • Assistance with bill paying
  • Information and referrals
  • Supportive counseling
  • Coordination of services and resources, including referrals to high quality caregivers, homemakers, home health care, adult day care, and senior residential options

Evaluations are held wherever you are (the hospital, your home, a nursing home). For some families, all that is needed is information and direction. Other situations require on-going contact. JFS care management staff is here to provide whatever is needed.
